Many situations do not necessitate the use of an emergency response as the best possible course of action, but a patient calling after hours is often directed towards the 911 system as a fallback.

One goal of SmartCare is to provide a healthcare option for patients who may otherwise be directed to use the 911 system for an urgent (not emergent need).

SmartCare offers an efficient, cost effective, and medically appropriate patient-centered healthcare option.

Home Evaluation

SmartCare Paramedics perform a thorough home safety assessment to identify risk factors that may cause subsequent health concerns. Assessment inside the home includes living room, kitchen, stairs, bedrooms, and bathrooms. Assessment outside the home includes sidewalks, driveways, and porches. This complete assessment is included in the PCP reporting.

PCP Connection

SmartCare Paramedics stay connected with the PCP to report clinical findings and discuss the most appropriate care plan. PCPs can update the patient's medical record remotely and have complete oversight of the patient's care throughout the process. This unique partnership delivers the highest level of care and the most efficient workflow.

Medications

SmartCare paramedics provide a number of services to support the patient’s path to recovery after a hospital visit. Proper medication administration & monitoring is a critical component to the patient’s wellness and recovery, which is why even SmartCare visit involves a detailed review of prescribed medications. Through detailed medication reconciliation and discussion with the patient’s PCP, you can be confident that the patient taking the right medication, in the right dosages, and at the proper intervals.  

Intervention

ECG MONITORING

12- lead acquisition and transmission using Zoll Technology. The Zoll X-Series also allows SmartCare paramedics to continuously monitor pulse oximetry, capnography, and blood pressure.

OXYGEN THERAPY

SmartCare paramedics can deliver all levels of oxygen therapy including supplemental care via nasal cannula, CPAP or BiPAP delivery devices, nebulized medications and deep tracheal suctioning.

Point of Care Testing

SmartCare vehicles are supplied with an iSTAT device capable of doing on-site CLIA-waived lab analysis.

Typical testing includes a CHEM-8 panel (sodium, potassium, chloride, ionized calcium, total CO(2), glucose, BUN and creatinine). Other CLIA-waived testing is available. Results are reported to the PCP in minutes.

Additional testing options include: simple blood glucose, finger-stick testing, rapid flu and strep testing, and urine analysis. If the necessary testing cannot be completed at the patient’s side, SmartCare paramedics will collect blood samples and deliver them to an affiliated lab for further testing, eliminating the need for a courier service.

WOUND CARE

SmartCare paramedics have been trained in basic wound care and work with long-term care providers to monitor progress and provide care to patients recovering from pressure wounds, etc.

MEDICATION RECONCILIATION/REVIEWS

SmartCare paramedics are trained to complete medication reconciliation/reviews with the patient under the direction of their PCP. If medications need to be obtained at a local pharmacy, SmartCare paramedics can pick-up/deliver medications to patients unable to obtain the proper medications themselves.

NEW TECHNOLOGIES

SmartCare paramedics now have access to ultrasound and X-ray diagnostics, COVID testing resources, and new therapies and diagnostics are added as needed.
